Job summary

Enzo Health is seeking a Forward Deployed Clinician to embed with home health agencies, leading onboarding from contract to full live adoption. You will learn each agency’s referrals, payer mix, QA processes, and reconcile them with Enzo EHR and the full product suite, then train all roles from field clinicians to back-office staff.

You bring 5+ years of back-office home health experience, hands-on OASIS and coding expertise, and a customer-first mindset.

Qualifications

  • 5+ years in home health back office: OASIS review/scrubbing, ICD-10 coding, QA and chart audit, or patient care management.
  • Current fluency in referral-to-billing lifecycle: intake, OASIS‑E, ICD-10 coding, QA, plans of care, orders, and claims.
  • Working knowledge of PDGM, LUPAs, HHVBP, and CMS Conditions of Participation.
  • Experience with at least one major EHR: reviewer, care manager, or super‑user.

Responsibilities

  • Own end-to-end onboarding for each new agency — kickoff, configuration, data migration, training, go-live, and early adoption.
  • Map agency referral-to-billing lifecycle and configure Enzo to fit it.
  • Validate data migrations from legacy EHRs with chart reviews.
  • Deliver role-based training for field clinicians, intake staff, coders, QA reviewers, billers, and agency leadership.
  • Audit early documentation and coding output to catch OASIS inconsistencies and compliance gaps.
  • Monitor adoption and revenue-cycle health in the first weeks after go-live.
  • Serve as an internal home health SME to inform product evolution (OASIS‑E, PDGM, HHVBP, CoP).
  • Turn learnings into repeatable playbooks for faster future implementations.
  • Partner with Sales and Customer Success for smooth handoffs.

The Role

You have lived the back office of home health. You have taken an OASIS assessment from a field clinician and turned it into a clean, accurate, billable episode. You have scrubbed charts, queried clinicians, fixed diagnosis coding, chased physician orders, and made sure the claim went out right the first time — because you were the one accountable when it didn't. Maybe you started in the field doing admissions and visits, then moved into QA, coding, OASIS review, or patient care management. That is exactly the person this role is built for.

As Enzo's Forward Deployed Clinician, you embed with each new agency and own their journey from signed contract to fully live and adopted. You are the person who turns Enzo's promise into results on the ground: you learn how the agency actually runs — its referrals, its payer mix, its QA process, and the problem charts — and you configure Enzo EHR and the full product suite (Intake, Scribe, QA, EHR) around that reality. Then you train every role in the building, sit with the back‑office team through their first episodes, and stay until the agency is performing.

This is not a generic software implementation job. Your credibility comes from having done the work our customers do every day. When a QA nurse pushes back on a workflow, you can answer from experience. When a coder asks why Enzo suggested a different primary diagnosis, you can walk through it. When an administrator worries about cash flow during transition, you know exactly which parts of the revenue cycle to watch.

What You'll Do
  • Own end-to-end onboarding for each new agency — kickoff, configuration, data migration, training, go-live, and early adoption — as their primary point of contact.
  • Map each agency's referral-to-billing lifecycle and configure Enzo to fit it: workflows, service lines, payer mix, and compliance requirements. You recommend the right setup from experience, not just replicate what the agency had before.
  • Validate data migrations from legacy EHRs (Homecare Homebase, WellSky/Kinnser, MatrixCare, Axxess, Netsmart, and others) with a chart reviewer's eye — patient records, OASIS assessments, plans of care, orders, and billing data.
  • Deliver role-based training for field clinicians, intake staff, coders, QA reviewers, billers, and agency leadership — taught by someone who has actually held those responsibilities.
  • Audit early documentation and coding output the way you would in a QA seat: catch OASIS inconsistencies, coding issues, and compliance gaps before they ever reach a claim.
  • Monitor adoption and revenue-cycle health in the first weeks after go-live, stepping in proactively where teams need reinforcement to hit documentation, intake, coding, and QA targets.
  • Serve as an internal home health subject‑matter expert, helping Product and Engineering reason about OASIS‑E, PDGM, HHVBP, and Conditions of Participation as the product evolves — your field reports shape the roadmap.
  • Turn what you learn into repeatable playbooks so every implementation after yours gets faster and better.
  • Partner with Sales and Customer Success for smooth handoffs into and out of onboarding.
What You Bring
  • 5+ years in home health with substantial time in the back office: OASIS review/scrubbing, ICD-10 coding, QA and chart audit, clinical documentation review, or patient care management. You have been personally accountable for documentation accuracy and reimbursement outcomes.
  • Deep, current fluency in the referral-to-billing lifecycle: intake and eligibility, OASIS‑E, ICD-10 coding, QA review, plans of care and physician orders, Notice of Admission, and claims.
  • Working command of home health payment and regulatory frameworks — PDGM, LUPAs, HHVBP, CMS Conditions of Participation, Medicare/Medicaid and managed care, and Review Choice Demonstration where applicable.
  • Daily-driver experience in at least one major home health EHR (Homecare Homebase, WellSky/Kinnser, MatrixCare, Axxess, Netsmart) as a reviewer, care manager, or super‑user. You know where the bodies are buried in these systems.
  • Clear, confident communication with every audience in an agency — you can teach a field nurse, challenge a coder, and brief an owner, and be credible with all three.
  • Comfort with AI‑driven and automated workflows, and the ability to explain them plainly to non‑technical clinical staff.
  • The organization and follow‑through to keep several agencies moving at once. Formal software‑implementation experience is not required — we will teach you the implementation craft; we cannot teach a decade in home health.
  • A customer‑first mindset and a genuine drive to see agencies succeed.
Nice to Have
  • An active RN license (strongly preferred), especially with home health field experience — admissions, case management, or visit work before moving into the back office.
  • OASIS and coding certifications: COS-C, HCS‑O, HCS‑D, BCHH‑C, or Fazzi OASIS certification.
  • Vendor-side experience — chart review, coding services, or clinical review at a home health software or services company.
  • EHR implementation, data migration, or system‑admin experience at an agency or vendor.
  • Prior experience at a high‑growth healthcare technology company.
