Job summary

Family Care of Hartford Corp is seeking a passionate Family Nurse Practitioner to join our Hartford, CT healthcare team. You will provide comprehensive primary care across all ages in a collaborative clinical environment.

Required credentials include FNP certification (AANP/ANCC), CT APRN license, and MSN/DNP with DEA privileges. 3–5 years in primary care preferred; strong EHR skills and patient-centered approach are essential.

Qualifications

• Active FNP certification (AANP/ANCC)
• Connecticut APRN license in good standing
• MSN or DNP from an accredited program
• DEA license and prescriptive authority in CT
• 3–5 years in primary care/family medicine preferred
• Proficiency with EHR systems
• Strong interpersonal and patient-centered communication skills
• Ability to work independently within a multidisciplinary team

Responsibilities

• Provide comprehensive primary care to patients of all ages
• Perform physical exams and health screenings
• Diagnose and treat acute and chronic illnesses
• Develop and manage individualized care plans
• Prescribe medications per state regulations
• Educate patients on preventive care and wellness
• Maintain accurate EHR documentation
• Collaborate with physicians and care teams for continuity of care
