Qualifications

• Graduate of an approved Nurse Practitioner or Physician Assistant program.
• Experience with and demonstrates patient centered care.
• Strong organization, prioritization, and time management skills in a fast-paced environment.
• Detail oriented with high degree of accuracy.
• Proficiency using Microsoft Office applications; willingness to learn database applications.
• Excellent verbal and written communication skills.
• Strong professional skills with ability to interact calmly and empathetically with diverse populations.
• Outstanding interpersonal skills with ability to de-escalate situations and work in teams.
• Licensure: Licensed NP in IA or PA certification; DEA registration; state Board of Pharmacy (CSA) registration; BLS; Mandatory Reporter Training; valid driver’s license and insurance.

Responsibilities

• Provides medical care per PHC standards, takes histories, conducts exams, orders tests, writes orders and prescriptions, performs procedures, and educates patients and families.
• Maintains and interprets electronic medical records; ensures complete documentation within PHC timelines.
• Handles correspondence, referrals, prior authorizations, refills, and letters to patients and providers within PHC timeframes.
• Participates in process improvement and quality measures; collaborates with Patient Care Team for effective function.
• Ensures compliance with regulatory requirements including risk management, medical records, accreditation, and federal/state/local regulations.
• Leads, mentors, trains, and educates staff, students, and residents; participates in research as desired.
