You will collaborate with clinicians to coordinate care, develop discharge plans, and ensure safety, quality, and compassionate patient experiences across the continuum of care.

Qualifications

• Graduate of an accredited School of Nursing; BSN preferred.
• Current Texas RN license or compact license.
• Basic Life Support certification required.
• Experience levels span from new grads to 10+ years.
