Qualifications

• Active license & current ANCC or AANP board certification required.
• CPR/BLS certification required at time of start date.
• Independent practice provider preferred, where applicable per State regulation.

Responsibilities

• Practice relationship-based medicine including prevention and chronic disease management.
• Remain flexible to cover shifts for absent teammates.
• Work autonomously while ensuring continuity across various teams.
