Job summary

Jane Pauley Community Health Center in Indianapolis, Indiana, is seeking a Licensed Practical Nurse (LPN) - Float to support our clinics. You will perform patient assessments, administer medications, coordinate lab work, document in the EHR, and help ensure efficient patient flow across sites.

Qualifications

• High school diploma or equivalent is required.
• Current, unrestricted LPN license in Indiana.
• Minimum 1–2 years of clinical nursing experience.
• Experience in primary care or community health preferred.
• BLS certification maintained throughout employment.

Responsibilities

• Perform patient assessments, obtain vital signs, and provide clinical interventions within scope.
• Float between JPCHC clinics to meet operational and patient care needs.
• Triage patients by phone or in person to determine next steps.
• Support chronic disease management and population health initiatives.
• Administer immunizations, medications, and treatments per orders.
• Educate patients on medications, disease management, and preventive care.
• Assist with patient flow and collaborate with care teams to optimize operations.
• Document care accurately in the EHR and coordinate follow-up for labs/imaging.
• Participate in care team huddles and quality improvement activities.
• Precept and mentor new clinical support staff.
