American Senior Communities is seeking a Float LPN for its Clinical Resource Group, covering multiple facilities in the Fort Wayne area. The role offers full-time, part-time, and PRN options with a $40/hour rate and a $5,000 sign-on bonus for qualifying FT/PT applicants.

The CRG Float Pool provides travel across facilities, a consistent monthly schedule, and comprehensive benefits including medical/dental/vision insurance, 401k, and tuition assistance for certifications.

Qualifications

• Active LPN license in Indiana and minimum 2 years LTC nursing home experience.
• Willingness to travel locally within ~50 miles.
• Strong teamwork and adaptability.

Responsibilities

• Support nursing teams with staffing needs and resident care.
• Deliver high-quality resident care across multiple facilities.
• Document and communicate changes to care plans and team.
• Participate in CRG Float Pool processes and promote continuity of care.
