Job summary

Monogram Health is seeking compassionate Advanced Practice Providers to deliver in-home care for patients with multiple chronic conditions. You’ll work within an integrated team and provide continuity of care in homes and via telehealth.

This role offers a value-based, patient-centered care model with opportunities to grow clinical skills, earn performance bonuses, and participate in CEU/CME reimbursement. Travel to patient homes is required in the Austin–San Antonio area.

Qualifications

  • Active, unrestricted NP/PA license or RN/NP license
  • Board certification (NP: ANCC/AANP | PA: NCCPA)
  • Current unrestricted DEA certificate or eligibility
  • Minimum 2 years direct patient care experience
  • Field-based role requires travel to patient homes and reliable transportation
  • Knowledge of HIPAA and OSHA regulations
  • Experience with EHR systems (Athena, Salesforce, Epic)
  • Background in managed care/population health is preferred

Responsibilities

  • Provide evidence-based, patient-focused care in patients' homes
  • Perform comprehensive assessments, wellness exams, and follow-ups
  • Educate patients and caregivers on disease management and medications
  • Participate in virtual care team meetings
  • Ensure compliance with state board requirements and best practices

This role workswithin an integrated multidisciplinarycareteam (Clinical, Behavioral Health, Pharmacy, Administrative),establishinglong-term patient relationshipsandprovidingcontinuity of caretocomplex patients in their homesand via telehealth.The Advanced Practice Providerhas the opportunitytobe part of a value-based, patient-centered care model, providingquality driven managed care with apopulationhealth focus.

    • The Float APP will be required to provide in-home care to patients in the following locations: Austin, New Braunfels, and San Antonio. This role offers a clustered schedule, meaning you’ll see patients in one area per day rather than traveling long distances between cities.
    • Patient visits occur 3-4 days/weekinthe patients’ homewith 1-2 days/weektelehealthvisits(variesby market)
    • No on‑call, overnights, weekends, including paid company holidays

Responsibilities
  • Provide evidence‑based, patient‑focused care that reduces avoidable hospitalizations and improves quality of lifedirectly in the patient’s home
  • Perform comprehensive assessments, including initial evaluations, annual wellnessexams,follow‑ups,acute urgent visits,andpost‑dischargevisits.
  • Educate patients and caregivers on disease management, preventive care, medications, and diagnostic testing
  • Participate inrecurringvirtual integrated care team meetings
  • Engage in quality assurance activities, including audits and documentation reviews, to ensure compliance with state board requirements and clinical best practices
  • May practice in a variety of care settings beyond the patient’s home, including skilled nursing facilities (SNFs), assisted living facilities (ALFs), hospitals, and other affiliated clinical settings, as applicable.
  • Perform point‑of‑care testing (A1C and Spirometry), prescribe medications, order tests, and collaborate with patient’s Monogram physician
  • Coordinate care with PCPs, specialists, and ancillary services
  • Complete documentation accurately andin a timely manner
PositionRequirements
  • Active and unrestricted RN and NP license, or Physician Assistant license
  • Board certification (NP: ANCC/AANP | PA: NCCPA)
  • Current and unrestricted DEA certificate, or eligibility to obtain
  • Minimumof two (2)years of direct patient care experience
  • Field‑based role requiringtravel to patient homes, reliable transportation, valid driver’s license, and auto insurance
  • Demonstrated initiative and independent, critical thinking, and decision‑making skillsKnowledge ofand compliance withfederal/state regulations (HIPAA, OSHA)
  • Prior experience with EHR systems (e.g.Athena, Salesforce, Epic)
  • Previousexperience conducting annual wellness or comprehensive visits preferred
  • Background in managed care, population health, or value‑based carepreferred
