Job summary

Stellar Health Group in Walpole, MA is seeking a licensed Physical Therapist to join our SNF therapy team. We offer flexible opportunities including Full-Time, Part-Time and Per Diem weekends and weekdays, with newly adjusted competitive wages.

You'll evaluate and treat residents, develop individualized plans of care, collaborate with interdisciplinary staff, document per guidelines, and participate in family conferences to support recovery and discharge planning.

Qualifications

• Current MA State License as a Physical Therapist.
• Flexible to be part of a dynamic and cooperative team.
• Ability to plan individualized treatment plans and goals.

Responsibilities

• Provide physical therapy services to the facility, promoting excellence in rehab care and interdisciplinary cooperation.
• Evaluate patients in a timely manner per state, payer source, or facility requirements.
• Screen patients upon admission or when requested by the director.
• Develop an appropriate plan of treatment and goals for each patient.
• Provide treatment as per the plan after patient evaluation.
• Communicate with other disciplines and staff according to the treatment plan.
• Provide routine documentation according to Medicare, facility, and payer source guidelines.
• Participate in family conferences when requested.
• Submit all required documentation and treatment charges timely, per policy.
