Qualifications

Must have active LPN/LVN license in the state of practice.
Compact licenses must be transferred within 90 days.
Current CPR certification (hands-on component) — assistance available after hire.
TB skin test current within last 12 months.
Six months nursing hands-on experience preferred but not required.

Responsibilities

Provide in-home nursing care to assigned patients.
Document care using a tablet and electronic records.
Collaborate with family and clinicians to achieve positive outcomes.
Adhere to infection control and safety protocols.
Travel to patient homes and coordinate with local support.
