Interim HealthCare Inc. is seeking an LPN Visit Nurse to provide in-home, patient-centered care under a formal plan of care. You will perform assessments, monitor progress, and coordinate with other providers to adjust treatment as needed.

Qualifications

• Graduate of a practical nursing program with current LPN/LVN license in the practice state.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites.
• Two years of experience as an LPN in home health or similar setting.

Responsibilities

• Provide in-home direct patient care per care plan and nursing scope of practice.
• Complete skilled assessments and deliver patient-centered home care.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient status and collaborate with providers to adjust care.
• Complete clinical documentation per agency protocol and Medicare guidelines.
• Educate patients and families involved in care.
• Help reduce readmissions by promoting preventive care and continuity.
