Responsibilities include medication administration, treatments, documentation, monitoring residents for changes in condition, and partnering with RNs, CNAs, and leadership to meet resident needs.

Qualifications

• Current Ohio LPN license is required.
• Experience in long-term care or skilled nursing preferred.
• Strong communication and documentation skills.
• Dependable, compassionate, and able to work well as part of a team.

Responsibilities

• Administer medications and provide treatments.
• Document resident status and care actions clearly and timely.
• Monitor residents for changes in condition and escalate as needed.
• Collaborate with RNs, CNAs, and leadership to meet resident needs.
