Job summary

Aveanna Healthcare is seeking a Licensed Practical Nurse (LPN) to provide and document skilled nursing care in patients’ homes under RN supervision. The role supports families, maintains confidentiality, and adheres to care plans and physician orders.

The LPN will participate in ongoing education, use infection control practices, and document care activities. Flexible scheduling with full-time, part-time, and PRN shifts is offered, with opportunities for skill advancement.

Qualifications

• Graduate of an accredited school of nursing.
• Current, unrestricted state license as a Licensed Nurse in the state of practice.
• Current CPR certification.
• Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures.
• Six months of pediatrics experience preferred but not required.

Responsibilities

• Delivery and coordination of quality patient care in accordance with physician orders.
• Observe and assess patient condition and care needs and report changes to supervisor or physician.
• Document all activities, assessments and nursing actions in a timely manner.
• Participate and update the nursing care plan.
• Take appropriate nursing action to achieve expected outcomes.
• Recognize changes in patient needs and implement care to prevent or reduce risk.
• Ensure compliance with Aveanna policies and state/federal regulations.
