Job summary

SYNERGY MEDICAL SERVICES LLC is seeking a Per Diem Registered Nurse to join our home health team in Pensacola and surrounding areas. The RN will create and manage personalized care plans, observe patient conditions, and communicate findings to the care team with compassion and accuracy.

Primary duties include coordinating with colleagues, administering medications, documenting changes, and ensuring adherence to OASIS and Medicare guidelines.

Qualifications

• Currently licensed RN in state of employment
• Minimum of 1 year experience as a Registered Nurse in Home Health care
• Previous experience with OASIS and Medicare guidelines/COPs preferred
• Excellent interpersonal and client care skills
• Valid driver’s license and reliable transportation
• First aid/CPR certification required

Responsibilities

• Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
• Administer medications and monitor patients for side effects or reactions
• Document any changes in patient conditions and report on changes
• Maintain accurate records of patient care and discuss observations with supervisor
