Job summary

Qualifications

• Minimum of one year of recent OT experience.
• Prior home health experience preferred but not required.
• Experience with EMR documentation preferred.

Responsibilities

• Provide skilled home visits within designated territories.
• Perform comprehensive evaluations of functional abilities including ADLs and IADLs.
• Develop individualized treatment plans to improve functional outcomes.
