Qualifications

Responsibilities

• Care for seniors and those with illness, injury or chronic disease in patients' homes.
• Assist with daily activities: bathing, toileting, dressing, grooming, hygiene, nutrition.
• Assist with ambulation, transfers and range of motion exercises.
• Check vitals, monitor intake/output, document changes.
• Collaborate with home health nurses and aides to follow care plans.
• Observe patients and notify supervisor of changes in care or needs.
