Job summary

Avera Sunrise Manor in Minnesota is seeking qualified nursing staff to deliver resident care as part of a multidisciplinary team. Qualifications

• Active RN license in Minnesota; or LPN license in Minnesota, Upon Hire.
• One year of RN/LPN experience required upon hire.

Responsibilities

• Practices nursing care under the supervision of a registered nurse, advanced practice registered nurse, licensed physician, or other health care provider.
• Collects data and conducts a focused nursing assessment of the health status of a patient.
• Participates with other health care providers in the development and modification of the patient-centered health care plan.
• Implements nursing interventions within a patient-centered health care plan.
• Assists in the evaluation of responses to interventions.
• Provides for the maintenance of safe and effective nursing care rendered directly or indirectly.
• Advocates for the best interest of the patient.
• Communicates and collaborates with patients and members of the health care team.
• Assists with health counseling and teaching.
• Delegates and assigns nursing interventions to implement the plan of care.
