Millennium Home Care is seeking a compassionate Registered Nurse to join our home health team on a PRN basis in Naples, FL. You will conduct patient assessments, develop care plans, educate families, and provide direct patient care in patients' homes.

Qualifications

• Current and valid Registered Nurse (RN) license in Florida.
• Minimum 1 year of hospital or skilled nursing facility experience.

Responsibilities

• Conduct comprehensive patient assessments including vital signs and health status.
• Develop individualized care plans addressing needs and goals; manage medications and wound care.
• Educate patients and families on disease management and self-care techniques.
• Provide direct patient care including wound care, injections, and IV therapy as needed.
• Collaborate with physicians and other healthcare professionals for coordinated care.
• Maintain accurate documentation of assessments, interventions, and progress notes.
