Job summary

Ambercare Hospice in Alamogordo, NM seeks a compassionate PRN Registered Nurse Case Manager to provide expert clinical care and support to patients and families, guiding plans of care with dignity.

This role offers flexible scheduling, autonomy in coordinating visits, competitive pay plus mileage, and opportunities to grow within a mission-driven hospice team. You will collaborate with the interdisciplinary team and document care plans timely to ensure high-quality patient outcomes.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed RN in the state of practice.
• Maintains a current CPR certification.
• Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and general public.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assist with patient and family caseload across assessment, eligibility, admission processing, planning, implementation and evaluation of the nursing process.
• Obtain data from patient physical, psychological, social and spiritual factors affecting care and coordinate with the interdisciplinary team.
• Coordinate with attending physician and other agencies to optimize care.
• Provide visits to reassess patient needs and update the plan of care.
• Perform nursing procedures as directed by physician and provide end-of-life support as needed.
• Maintain accurate patient records, charts, and timely care plans.
