Job summary

HealthPro Heritage is seeking a Licensed Physical Therapist Assistant (LPTA) for a PRN position at Grafton County Nursing Home in Haverhill, NH. The facility offers a flexible schedule and a 135-bed, 4-star skilled nursing setting with competitive benefits.

You will assist in implementing treatment plans, monitor progress, educate patients/families, document care, and collaborate with the care team to achieve optimal outcomes in a supportive, therapist-led environment.

Qualifications

Associate’s degree in Physical Therapist Assisting from an accredited program.
Valid state licensure as a Physical Therapist Assistant, or license eligible.
Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.

Responsibilities

Patient Care: Assist in implementing treatment plans designed by Physical Therapists, providing direct patient care including exercises and modalities.
Treatment and Education: Monitor patient progress, report observations to the Physical Therapist, and educate patients and families on protocols and self-care.
Documentation: Accurately document patient treatments, progress, and responses per facility policies.
Collaboration: Work with Physical Therapists and other staff to ensure coordinated care.
