Qualifications
Graduate of an accredited Physical Therapy program (Master’s or Doctorate).
Current, active Physical Therapist (PT) license in Texas.
At least 1 year of experience in IDD, home health, pediatrics, or geriatrics.
Strong knowledge of therapy practices and person-centered care.
Excellent communication and documentation skills.
Responsibilities
Conduct physical therapy evaluations and develop individualized treatment plans.
Provide skilled physical therapy services in home and community-based settings.
Implement interventions to improve mobility, strength, coordination, and functional independence.
Collaborate with interdisciplinary team (IDT) members, caregivers, and family members.
Monitor client progress and modify treatment plans as needed.
