Job summary

Human Touch Home Health is seeking a PRN Physical Therapist to serve patients along the I-95 Corridor from Triangle to Springfield, Virginia. You will evaluate patients in their homes and create personalized care plans, delivering therapeutic exercise and mobility training with a focus on independence.

Qualifications

• Active Virginia Physical Therapist License is required.
• Graduate of an accredited Physical Therapy program.
• Valid Driver's License and reliable transportation.
• Strong clinical assessment and communication skills.
• Ability to work independently in patients' homes.
• Home Health experience preferred, but not required.
• WellSky experience is a plus.

Responsibilities

• Complete physical therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Provide therapeutic exercise, gait, balance, transfer, and mobility training.
• Help patients improve strength, endurance, and functional independence.
• Assess home safety and identify fall risks.
• Educate patients and caregivers on mobility, safety, and home exercise programs.
• Monitor patient progress and adjust treatment plans as needed.
• Complete reassessments and discharge planning.
• Coordinate care with physicians, nurses, therapists, and other team members.
• Complete accurate and timely documentation using WellSky.
