Touching Hearts at Home is seeking a compassionate Physical Therapist (PRN) to provide skilled PT services to patients in their homes within the Fredericksburg area. You will evaluate, develop treatment plans, and implement therapies to improve mobility and independence.

The role requires current PT license, DPT preferred, CPR/BLS, valid driver's license, and ability to travel between patient homes. Experience in home health or outpatient rehab is a plus; strong documentation is essential.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Virginia.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Ability to travel between patient homes.

Responsabilités

• Conduct home PT evaluations.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercises, gait and balance training.
• Assess progress and adjust plans as needed.
• Educate patients and caregivers on home exercise and safety.
• Document in EMR per agency policy.
• Communicate with physicians and care team regarding patient status.
• Participate in care coordination as needed.
• Ensure compliance with home health standards.
