Job summary

Responsibilities

• Perform in-home patient assessments.
• Develop and follow individualized care plans.
• Provide skilled nursing care (wound care, meds, catheter care, chronic disease management).
• Educate patients and caregivers on home care and safety.
• Monitor patient progress and update care plans.
• Complete timely and accurate documentation.
