Job summary

BrightStar Care in Texas is seeking a licensed Physical Therapist to provide one-on-one home health services in the DFW area, with a PRN schedule.

You will assess, plan, and monitor rehabilitation programs, educate patients and families, and document progress while collaborating with a multidisciplinary team to deliver compassionate, high-quality care in patients' homes in Lucas and surrounding areas.

Qualifications

• Valid Physical Therapist license in the state of Texas.
• Experience in home health or outpatient settings.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a flexible schedule.

Responsibilities

• Assess patients' physical abilities and plan individualized rehabilitation programs.
• Implement and monitor treatment plans to ensure progress toward goals.
• Provide education and support to patients and families regarding treatment protocols and home exercises.
• Document patient progress and communicate effectively with clinical staff and other healthcare providers.
• Work collaboratively with our team to provide high-quality, patient-centered care.
