Job summary

MedCap Health in the United States is seeking a Long Term Care Doctor – Post-Acute Care Physician to join our physician-led team. The role offers flexible scheduling—full-time, part-time, or custom hours—along with autonomy and leadership opportunities in post-acute care.

You will lead care teams in skilled nursing facilities, coordinate with NPs and specialists to deliver hospital-level care in a long-term setting, and ensure continuity, quality, and compassionate patient interactions.

Qualifications

• Board certification or board eligibility in internal medicine, critical care, family practice, or geriatrics.
• Leadership experience or hospitalist background preferred.

Responsibilities

• Lead care teams in skilled nursing facilities and manage complex post-acute cases.
• Collaborate with nurse practitioners and specialists to deliver hospital-level care in a long-term setting.
• Ensure continuity, quality, and compassionate patient interactions.
