Fortcom Health Services is seeking a dedicated and compassionate Per Diem Registered Nurse (RN) / LPN to support our Adult Family Care (AFC) and Adult Foster Care program. The RN will perform in-home visits every two months and complete required documentation for a limited caseload, collaborating with the AFC team to ensure eligibility and quality home-based care.

The role offers flexible hours, mileage reimbursement, and meaningful community-based nursing work across assigned MA areas.

Formación

• RN license in good standing with MA Board of Registration in Nursing.
• Associate or BSN preferred.
• Home health/community nursing experience a plus.
• Strong clinical documentation and organizational skills.
• Bilingual in Spanish or Haitian Creole is a plus.

Responsabilidades

• Conduct in-home visits every two months and complete clinical documentation for a limited caseload.
• Develop, review, and update AFC plans of care.
• Train, evaluate, and supervise AFC caregivers with Program Director.
• Review physician summaries and ensure care aligns with clinical recommendations.
• Monitor health status and report changes to primary care physician.
• Submit semiannual health status reports to primary care physicians.
• Plan for and coordinate member discharges from AFC program.
• Deliver caregiver training on health, safety, and aging.
