Job summary

Karing Hearts Home Health is seeking a compassionate Per Diem RN to join our home health team in Imperial, CA. This role offers flexibility and the opportunity to make a meaningful difference in patients’ lives within their own homes.

Ideal candidates are licensed RNs in California, with CPR/BLS certification and at least 1 year of clinical nursing experience. Strong communication and critical thinking skills are essential for coordinating care with physicians and the team.

Qualifications

• Active California RN license in good standing.
• Minimum 1 year of clinical nursing experience; home health experience preferred.
• Current CPR/BLS certification required.
• Valid California driver’s license and reliable transportation.

Responsibilities

• Conduct skilled nursing assessments and develop individualized patient care plans in a home health setting.
• Administer medications, wound care, IV therapy, and other clinical treatments as ordered.
• Monitor and evaluate patient progress, adjusting care plans as needed.
• Educate patients and family members on disease management, medications, and self-care techniques.
• Collaborate with physicians, therapists, and interdisciplinary team members to coordinate patient care.
• Accurately document clinical notes and patient records in compliance with regulatory standards.
• Ensure patient safety and comfort while promoting independence and quality of life.
