Job summary

Family Pillars Homecare is seeking dedicated RNs and LPNs to provide skilled nursing care in patients' homes across Florida. Per diem shifts include intermittent visits, private duty coverage, and care plan coordination for licensed nurses able to travel locally.

Responsibilities include wound care, IV administration, medications, patient assessments, and collaboration with physicians. Strong emphasis on compliance, documentation, and timely, compassionate care.

Qualifications

• Active Florida RN license or LPN license (with Florida eligibility)
• Current CPR certification (BLS)
• Familiarity with EMRs (e.g., Axxess) and knowledge of PDGM & OASIS for RNs
• Reliable transportation and punctual, professional care

Responsibilities

• Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments
• Carry out shift-based care for patients needing extended clinical support
• Monitor and document patient condition and progress
• Collaborate with physicians and interdisciplinary teams to update care plans
• Maintain accurate records in HIPAA and CMS guidelines (OASIS for RN)
• Uphold infection control standards and agency protocols
