Job summary

Always Best Care Home Health is seeking a dedicated Registered Nurse (RN) to provide skilled nursing services to patients throughout East Honolulu. This per diem position offers the flexibility to build your own schedule while delivering one-on-one care in patients' homes.

We welcome applications from RNs passionate about delivering exceptional patient care in the home setting, with SOC experience preferred and opportunities to work in SOC admissions and other Medicare home health assessments.

Qualifications

• Active Hawaii RN license.
• Minimum one year of clinical nursing experience.
• Home health experience preferred but not required.
• Experience with OASIS SOC assessment preferred.

Responsibilities

• Conduct skilled nursing visits in patients' homes.
• Complete comprehensive assessments and reassessments.
• Develop and update individualized care plans.
• Provide patient and family education.
• Perform wound care, medication management, home infusions, disease management and other treatments.
• Coordinate care with physicians and interdisciplinary team members.
• Monitor patient progress and communicate changes in condition.
• Complete accurate and timely clinical documentation.
• Ensure compliance with Medicare home health standards and agency policies.
