Qualifications

Responsibilities
Conduct comprehensive initial assessments to determine individualized home care needs.
Develop, implement, and continuously evolve patient‑centered care plans.
Perform ongoing nursing assessments and adjust care plans in real time based on changing conditions.
Administer treatments, medications, and interventions as prescribed.
Empower patients and families through health education and hands‑on guidance.
Support proactive discharge planning to ensure seamless care transitions.
Maintain clear, timely, and accurate clinical documentation.
Collaborate closely with physicians, care coordinators, and interdisciplinary teams.
