Qualifications

• Certification of Hospice and Palliative Nursing (CHPN) preferred.

Responsibilities

• Perform palliative admissions and follow-up visits for hospice patients.
• Evaluate patient health history and develop care plans.
• Provide hospice care including physical assessments and eligibility evaluations.
