Heralds Home Health seeks a per diem Licensed Vocational Nurse (LVN) to provide in-home skilled nursing visits across Gardena, Hawthorne, Lawndale, Compton, Carson and surrounding communities.

Responsibilities include conducting visits, monitoring vitals, wound care, medication support, patient education, and clear communication with the supervising RN, along with thorough documentation. Independent work style and reliability are essential for success.

Qualifications

• Active California LVN license required.
• Home health experience preferred.
• Strong clinical skills.
• Dependable transportation.
• Excellent organizational skills.
• Ability to work independently in the field.

Responsabilités

• Conduct scheduled skilled nursing visits.
• Monitor vital signs and patient conditions.
• Provide wound care and ordered treatments.
• Assist with medication management and patient education.
• Communicate patient updates to the supervising RN.
• Maintain timely and accurate clinical documentation.

Connaissances
