Qualifications

• Active and unrestricted RN license required.
• Prior home care or field nursing experience preferred.
• Strong assessment and documentation skills needed.

Responsibilities

• Complete start-of-care assessments
• Conduct supervisory nursing visits
• Perform patient reassessments
• Complete post-hospitalization visits
• Provide on-call nursing visits as assigned
• Assess patients’ clinical conditions and care needs
• Report changes in patients’ conditions to the clinical team
• Complete all required visit documentation accurately and on time
• Maintain patient confidentiality and follow HIPAA requirements
• Comply with Affiliated Home Care policies and applicable nursing regulations
