Qualifications

• Must have one or two years of recent acute care experience in an institutional setting.
• Current licensure in California and CPR certification is required.
• Candidates with experience in home health preferred.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Acts as Case Manager and assumes responsibility to coordinate patient care.
