Job summary

JTC Home Health Inc. is seeking an experienced Per Diem Physical Therapist (PT) to join our team serving El Cajon, La Mesa, Santee and surrounding East County in San Diego.

You will work in patients' homes as part of an interdisciplinary team, delivering comprehensive PT services and ensuring safety and independence. Responsibilities include assessments, developing a Plan of Care, administering therapies, patient and caregiver education, and accurate EMR documentation.

Qualifications

• Current and unrestricted Physical Therapist (PT) license in the State of California.
• 1+ year of clinical experience as a Physical Therapist; Home Health experience preferred.
• Current CPR certification (AHA Basic Life Support).
• Valid driver's license and insured vehicle; travel across East County service area.

Responsibilities

• Conduct initial and ongoing assessments of patients’ functional status, mobility, strength, and home safety.
• Develop, implement, and document the Plan of Care (POC) with physician and supervisor.
• Provide skilled PT treatments (exercises, manual therapy, gait training) to restore function.
• Educate patients and caregivers on equipment, home exercise programs, and safety.
• Complete clinical documentation accurately using the EMR system.
