Job summary

Chapters Health System is seeking a compassionate Per Diem CNA to provide personal care and homemaker assistance under RN direction. You will perform defined nursing procedures and support patients and their caregivers in their homes, ensuring safety and dignity.

Applicants must hold an active CNA certification in the state and a current BLS card. A HS diploma or GED is required, with prior CNA or Home Health Aide experience preferred.

Qualifications

• Requires CNA certification in the state where employed.
• High school diploma or GED required; prior CNA/Home Health Aide experience preferred.
• Active BLS certification from American Heart Association or Red Cross.

Responsibilities

• Provide personal care and homemaker assistance under RN supervision.
• Perform defined nursing procedures per Plan of Care.
• Maintain patient safety and home environment during visits.
• Communicate observations and concerns to RN/Clinical Manager.
• Comply with state/federal regulations and company policies.
