Eunice LA PDN PCS, part of LHC Group, is hiring a Home Health Licensed Practical Nurse to provide direct nursing care in patients' homes under physician orders and to document care in the EMR. You will work under supervision of an RN or supervisor in the geographic service area.

Responsibilities include delivering high-quality services per state practice laws, coordinating care, and maintaining thorough patient records. LA-specific LPN licensure and CPR are required, with reliable transportation.

Qualifications

• Active LPN licensure in state of practice.
• Current CPR certification.
• Valid driver's license and access to a dependable vehicle or public transportation.
• Ability to serve diverse patient populations in home settings.

Responsibilities

• Provides high quality clinical services within the scope of practice and in coordination with the health care team.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
