Job summary

Phoenix Home Care & Hospice is seeking caregivers to provide compassionate, one-on-one home care for our special needs clients under the Healthy Children and Youth program.

You will handle activities of daily living, companionship, and support for clients with varying acuity. A reliable vehicle and ability to lift are essential, with flexible scheduling and opportunities for growth in a supportive team environment.

Qualifications

• Be at least 18 years of age.
• Have a valid driver’s license.
• Reliable vehicle with current auto insurance.
• Ability to lift 50 lbs.
• Ability to pass a background check.
• Ability to pass a drug test.

Responsibilities

• Assist with Activities of Daily Living (Bathing, Dressing, Grooming).
• Provide companionship and emotional support.
• Support clients with higher acuity needs, including transfers (gait belts, Hoyer lifts).
• Assist with feeding and meal preparation.
• Monitor and report changes in condition.
• Light housekeeping and medication reminders.
• Run errands.
