Job summary

Qualifications

• Education for new grads or PTs with limited or no Skilled Nursing experience.

Responsibilities

• Plan treatment to residents and patients to relieve pain, restore or improve function.
• Provide clinical support and instruct patients, families, and caregivers.
• Monitor patient response to treatment.
• Complete required forms and documentation in accordance with company policy and state/federal regulations.
• Consult with patients to learn about their physical condition.
• Develop treatment plans using various techniques.
• Create personalized fitness-oriented health care programs.
• Administer physical therapy treatments to relieve pain and improve mobility.
• Advise patients on exercise techniques and in-home treatment options.
