Qualifications

• Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license.
• Board Certification: Nurse Practitioners ANCC/AANPCB/PNCB/AACN; Physician Assistants NCCPA.
• Board Certified or Board Eligible: FM, IM, Geri, PM&R, ED.
• Excellent clinical judgment, communication, and patient care skills.
• Ability to work independently while collaborating within a multidisciplinary team.

Responsibilities

• Conduct six-hour facility rounds; rounds typically begin between 7:00 a.m. and 10:00 a.m.
• Manage acute and chronic conditions and develop treatment plans.
• Prescribe medications as appropriate within licensure.
• Collaborate with nursing staff and interdisciplinary teams to ensure quality care.
• Ensure compliance with clinical and regulatory standards.
• Provide patient-centered care in skilled nursing facilities in Mishawaka, IN.
