Acute Home Healthcare is seeking a Medical Social Worker (MSW) in Osceola County to join a dynamic team on a per diem basis. The role supports patients and families, participates in discharge planning, and coordinates social services across the agency.

MSW with Home Health experience is required. Per diem position with hourly pay of $85, offering flexible scheduling, local visits near your home, and strong clinical support.

Qualifications

• Master's degree from an accredited school of Social Work (CSWE).
• Bachelor's degree in social work, psychology, sociology, or related field.
• One year of Home Health experience.
• Ability to exercise initiative and independent judgment.
• Must be a team player.
• Level 2 Background screening.

Responsibilities

• Serve as a resource to employees, patients, physicians and allied healthcare providers.
• Participate in development of the plan of care.
• Prepare clinical and progress notes.
• Assist in understanding social and emotional factors related to health problems.
• Work with the family and participate in discharge planning.
• Document all patient/family services as required by policy.
• Plan and coordinate social services within the Agency.
• Maintain contact with community support groups and knowledge of Medicare Conditions of Participation for social work services.
