Job summary

Skyline Nursing and Rehabilitation is seeking a qualified Licensed Practical Nurse (LPN) to join our compassionate team in providing high-quality care to seniors. You will work in a supportive environment, administering care plans and collaborating with CNAs and other clinicians.

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing.
• Experience in skilled nursing or rehabilitation preferred.
• A compassionate, detail-oriented, and dedicated professional committed to excellence in patient care.

Responsibilities

• Develop and implement individualized treatment plans for residents.
• Supervise and support CNAs, including participation in their evaluations.
• Oversee the Falls Program and Pressure Sore Program to ensure resident safety and well-being.
• Provide essential nursing services and direct patient care.
• Conduct and document thorough medical evaluations on admission and throughout treatment.
• Perform additional duties assigned to support the best possible care for our residents.
