Job summary

Collage Rehabilitation Partners is seeking LPNs in Denver, Colorado, offering pay between $34–36 per hour. Qualifications

• 2 years experience as an LPN in a home care setting.
• Proof of current CPR certification.
• TB test (active, not expired).
• Valid driver’s license and reliable transportation.
• Ability to work a flexible schedule and travel locally.

Responsibilities

• Participate in the planning and coordination of total patient care.
• Accurately report and record the patient's condition and care.
• Prepare clinical/progress notes and summaries.
• Assist the patient with the activities of daily living.
• Provide and maintain a safe environment for the patient.
• Assist the physician and Registered Nurse in complex nursing situations.
