Job summary

A Path of Care Hospice in Norman, OK is seeking a Nurse Practitioner (PRN/full-time) to provide skilled hospice care across home health settings. You will conduct visits, perform exams, and document care plans in collaboration with the care team.

The role emphasizes flexibility, meaningful patient work, and a supportive environment with opportunities to contribute to care policies and staff education.

Qualifications

• Must be a Nurse Practitioner licensed in Oklahoma with a license free of sanctions.
• Experience in geriatrics or palliative care preferred.
• Experience in a certified hospice program is a plus.
• Field work required (95% of the time) with mobility to perform visits.

Responsibilities

• Perform visits, physical examinations, and Face to Face assessments.
• Document care plans and coordinate with the Administrator/Director and Medical Director.
• Collaborate with the IDT and Ethics Committee as needed; support surveys and in-services.
