PurposeCare is a leading home care and home health provider serving the Midwest, delivering care in patients' homes with a focus on safety and independence. We seek a licensed physical therapist to join our growing team and bring expert care directly to clients in their communities.

You will conduct skilled PT visits in home settings, tailor plans to individual needs, and help clients improve daily function and quality of life while working within a supportive, technology-enabled ecosystem.

Qualifications

• Current Physical Therapist license in the state you are applying.
• 1 year of physical therapist experience preferred; home health and OASIS experience preferred.
• Ability to perform tasks involving physical activity, including heavy lifting up to 50 lbs, bending, and standing.
• Valid Driver's License and access to reliable transportation.
• Ability to successfully complete a background check.

Responsibilities

• Provide skilled physical therapy visits in patients' homes.
• Address the full scope of a patient's needs with real-life therapy and holistic care.
• Collaborate with patients to improve daily living and recovery in their home environment.
