Qualifications

• Licensed Physical Therapist in Arizona.
• Graduated from a CAPTE-accredited program and passed state licensure exam.
• Minimum 2 years’ experience in home health (preferred).
• Current CPR certification.
• Driver’s license and reliable transportation.

Responsabilités

• Develop and update individualized care plans.
• Provide physical therapy services per physician’s plan of care.
• Assess functional status, mobility, and home environment.
• Educate patients and families on therapy techniques and equipment.
• Maintain accurate documentation and communicate progress to physicians.
