Qualifications

• Active Illinois LPN license
• Graduate of an approved practical nursing program
• Home health experience preferred
• Strong clinical assessment and documentation skills
• Ability to work independently in a home-based setting
• Excellent communication and interpersonal skills
• Ability to follow physician orders and plans of care
• Reliable attendance and punctuality
• Valid driver’s license and auto insurance when driving
• Authorization to work in the United States

Responsibilities

• Provide skilled nursing care in the client's home
• Assess and monitor patient condition within scope of practice
• Follow physician orders and the established plan of care
• Administer medications and treatments as ordered
• Monitor vital signs and document observations
• Identify changes and report concerns promptly
• Provide wound care and other skilled nursing interventions
• Educate patients and reinforce team instructions
• Communicate with physicians, RNs, supervisors, patients, and families
• Maintain accurate clinical documentation
• Complete visit notes and electronic documentation
• Follow EVV, infection control, and compliance requirements
• Protect patient confidentiality
• Participate in care coordination with the interdisciplinary team
• Provide compassionate care supporting safety, comfort, and independence
