Job summary

Concierge Home Care is seeking a Home Health Aide (HHA/CNA) for PRN visits in West Palm Beach, FL. You will support patients with personal care while promoting dignity and independence, traveling to homes throughout the day. Expected to document visits and communicate changes to the RN.

We offer flexible scheduling, pay-per-visit compensation, mileage reimbursement, and dictation tools to streamline charting. AHCA Level 2 background screening may be required.

Qualifications

• Completion of a Home Health Aide training program and competency evaluation meeting Medicare’s Conditions of Participation.
• Ability to travel to patients’ homes throughout the day.
• Valid driver’s license, reliable transportation and auto insurance.

Responsibilities

• Assist patients with personal care, bathing and activities of daily living.
• Document visits accurately and promptly per agency policy.
• Notify supervising RN of any changes in patient condition.
