Job summary

LiveWell Partners seeks compassionate Registered Nurses to work one-on-one with patients in their homes, establishing and overseeing all aspects of case management. You will create Plans of Care and align with physician orders to improve patient independence and satisfaction.

You will be part of a team of clinicians, delivering high-quality care with disciplined execution and a patient-centered approach.

Qualifications

• Minimum of 2 years of nursing experience with a preference for 2+ years in an acute or a post-acute care setting.
• Active State Registered Nurse licensure is required.
• Proof of current CPR certification and valid Driver’s license.

Responsibilities

• Assumes responsibility for assigned patient’s care from admission through discharge by effectively executing patient’s Plan of Care.
• Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources.
• Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.
• Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
• Participate in on-call rotation as reasonably assigned.
